Advanced Care uses a team-based approach that allows for in-home diagnostic and lab testing, treatment, disease management, and intensive care coordination during an episode to treat general medical conditions in a patient's home.   Reporting to the local Advanced Care Lead APP, the Orlando, FL Advanced Care Advanced Practice Provider leverage progressive clinical skills to deliver advanced level care to patients in their home. The Advanced Care APP will have extensive clinical duties, but also serve as a key component of an innovative and highly functional team, responsible for delivering high-quality care in novel care model which improves both patient and provider satisfaction.   #ADV

This is an opportunity to be part of an innovative team that is implementing a novel program to deliver care to patient’s homes as an alternative to hospitalization. Reporting to the Regional Director of Advanced Practice, the Advanced Care APP Ambassador will be responsible for working collaboratively with a team of Physicians, APPs, Dispatch Health Sherpa (Nurse Navigator), RNs, and other ancillary services to provide advanced level care for patients in their home as an alternative to going to the hospital. This APP will travel to Advanced Care markets nationally and participate in care delivery, program development and standardization across markets. Our Advanced Practice Providers team with DHMTs (Medical Technicians) and are equipped with everything needed to treat common to complex injuries and illnesses through comprehensive health assessments, including diagnosis, treatment, and outcome management, for patients of all ages. Our innovative model allows us to provide definitive care in the home, including point of care laboratory studies, minor procedures, splinting, wound care, suturing, IV fluid and medication administration.
