This is an opportunity to be part of an innovative team that is implementing a novel program to deliver care to patient’s homes as an alternative to hospitalization. Reporting to the Regional Director of Advanced Practice, the Advanced Care APP Ambassador will be responsible for working collaboratively with a team of Physicians, APPs, Dispatch Health Sherpa (Nurse Navigator), RNs, and other ancillary services to provide advanced level care for patients in their home as an alternative to going to the hospital. This APP will travel to Advanced Care markets nationally and participate in care delivery, program development and standardization across markets. As a member of DispatchHealth’s Transition to Practice Fall 2023 Program Cohort, you’ll work alongside other program fellows across the country to put your skills from your Nurse Practitioner/Physician Assistant training and advanced degree into practice. You’ll gain experience in an intensive nine-month training and learning program to prepare you to see patients independently and enable you for success. You’ll work side by side in your market with a preceptor and connect virtuall with your cohort colleagues across the country. Additionally, you will be trained and mentored by Advanced Practice leaders who will provide comprehensive education, training, and feedback on how to care for acutely ill patients as you participate in transforming healthcare, one patient at a time.   Interviews for the next cohort are starting soon and classes launch Fall 2023 -  September 11.

DispatchHealth has set out to create the most advanced and complete in-home care model in the world. DispatchHealth Advanced Care is novel health care delivery program, providing acute, hospital-level care in patient’s homes as an alternative to hospitalization. Advanced Care uses a team-based approach that allows for in-home diagnostic and lab testing, treatment, disease management, and intensive care coordination during an episode to treat general medical conditions in a patient's home.   Reporting to the local Advanced Care Lead APP, the Tacoma Advanced Care Advanced Practice Provider leverage progressive clinical skills to deliver advanced level care to patients in their home. The Advanced Care APP will have extensive clinical duties, but also serve as a key component of an innovative and highly functional team, responsible for delivering high-quality care in novel care model which improves both patient and provider satisfaction.   #ADV
